Long story made short…the 22% is an estimate based upon a survival analysis.

This type of analysis takes into account the data that does exist at the time point of interest (5yrs) and that data for some subjects (~50%) is no longer available by that time point. It is attempting to address the idea that some proportion of the patients for whom data is no longer available would have ‘survived’ (not had a recurrence) by that time point.
