I wanted to share my experience with Dr. Shomaf Nakhjo because I know how hard it is to find reliable info when you’re specifically looking for tissue-based, mesh-free hernia repairs.

I’m 40, and earlier this year I was diagnosed with two direct inguinal hernias. After researching mesh vs. tissue repairs, I decided I wanted a pure tissue repair performed by someone who truly specializes in it. After a lot of reading and comparing techniques, I ended up choosing Dr. Nakhjo in New Jersey for his “mesh-free” modified Shouldice repair. I actually traveled from Michigan to have him do the surgery, which should give you an idea of how confident I felt in his approach.

    Surgery Experience

The surgery went smoothly and took just under 2 hours for both sides. He uses two small incisions (about 2” each), and they’re healing very well with minimal cosmetic impact. Newton Medical Center was excellent; organized, supportive, and professional throughout. His office staff was also great to work with and responsive to questions.

Do you know what his modified Shouldice consists of? Did he cut the cremaster or any nerves?

He makes only 2” incisions, he uses Prolene sutures vs stainless. He only opens the cremaster up about 20% and attempts to leave all nerves intact.

    Return to the Gym

Around this 11-week point, I’ve begun getting back into weight lifting—but I’m doing it cautiously. I’m focusing mainly on seated-position exercises and controlled movements to avoid unnecessary strain on the repair while everything continues to heal. So far, no issues.

    Recovery

The first two weeks were more challenging than I expected. My left inguinal canal area would flare up with discomfort even after short walks. It wasn’t debilitating, but it was definitely noticeable and required patience. Thankfully, that gradually improved, and by the 3-week mark I felt about 90% normal doing normal daily tasks.

Now at 11 weeks post-op, things are continuing to progress. I still have:

    • Some remaining numbness in the groin area.
    • Slight pelvic bone discomfort on the left side after intimacy or workouts (Dr. said this is and can be normal, it’s where the sutures are anchored and take some time to settle down).

Both are very minor and seem to be slowly improving. The lower abdominal area itself feels strong and stable, which has been reassuring as I’ve started easing back into physical activity.
