  • Hernia on ultrasound no lump

    Hi I have had chronic pelvic pain for the last year, it feels like a red hot poker had been inserted in the inguinal area , worse when I am sitting and after activity, less when standing . I had an ultrasound that showed . It is totally ruling my life.
    , “ Ultrasound findings;, Rt groin examined with patient coughing,straining and performing, Valsalva., There is a reducible Rt inguinal canal hernia demonstrated measuring, 14.1mm x 9.3mm in diameter and containing mesenteric fat tissues., No femoral hernia noted., Multiple prominent benign looking reactive lymph nodes seen in Rt, groin., “
    I saw a surgeon who examined me and found no lump , he said I had no hernia and so could do nothing and the pain was caused by my other problems ( chronic UTI).
    any thoughts on this please, do I seek a second opinion or is he correct?

    The “groin pain, found a hernia on ultrasound, but no hernia on palpation test” a fairly common topic of discussion on this forum and it’d be great to hear surgeons chime in with their thoughts and experiences.

    I had to show both the ultrasound tech. and the doctor where i was feeling the slight bulge before it could be confirmed by both since it was present while standing.

    Just out of curiosity, are you a male or female? Are you thin and/or athletic, or overweight? When did the pain begin, and was there a particular event that you recall which started it? Is there anything that makes the pain better or worse?

    Many surgeons take the approach yours did of “no bulge / no palpation = no hernia”. Shouldice has that approach, as do many other doctors.

    The other commentators are correct in that many/most people with a groin hernia simply have a bulge with no pain. The hernias with pain are less common, and even more rare are the hernias that are painful but do not show up at all. That is also why it’s important to make sure that other causes of pain have been ruled out, because groin hernia surgery has a high risk of chronic pain in general and that should arguably be your biggest concern, though most surgeons focus on recurrence, which I think has it wrong. A recurrence is a nuisance, whereas chronic pain can be life changing. Particularly in the current era of mania regarding opioid usage and prescribing, chronic pain should be the most important outcome since nobody would need opioids or anything else for chronic pain if they did not have chronic pain to begin with.

    You may want to find someone who is an expert at diagnosing the many various potential causes of pelvic and groin pain, hernia included.

    Hernias usually cause no pain. Do research on hernia repair because it is not a simple procedure like most surgeons will say it is. Some go on there whole life not getting it repaired, there can be a lot of reasons for pelvic/groin pain. Hernia surgery can cause up to 30% chronic pain especially when mesh is used. A UTI would cause burning while urinating but not as bad a pain as you describe. Surgeons will usually tell you to get it taken care of as soon as you know you have a hernia but nowadays they are aware of how many people are getting chronic pain from what is told to be such a simple procedure and in my opinion it is not so nowadays they are watching and waiting with them. They even say that if you are in pain before surgery then the chances of you being in pain after getting it repaired are high. Look up about mesh and if I was you never go that route, it is not intended to be implanted in the human body and it has ruined many lives. If you are going to get it repaired down the road go to a hernia specialist not just a general surgeon. Try to find on that does natural tissue repairs because they use no mesh. I wish you the best!

    I’m not an expert, but people can go a long time dealing with an inguinal hernia. Looks like the ultrasound showed hernia possible small give the dimensions with no bulge I believe the doctors right. Doctor’s just not going to go cutting into with nothing to fix or repair. I’d go on a weight management program in case a bulge does develop it’s much easier to recover from surgery when managing weight. I hope the prescribed medications work for the UTI…..sounds like the right diagnosis to me. Good luck!
