In addition to a non-contrast pelvic MRI (using the Valsalva protocol) to detect an occult hernia, is there any benefit to following up immediately with a contrast MRI? What can a contrast pelvic MRI reveal in a male that a non-contrast pelvic MRI wouldn’t capture?

Nothing. Contrast MRI has no purpose for pelvic hernia.
