Operative findings..Direct and indirect right inguinal hernia…An ultrasound guided ilioinguinal block was preformed with 7ml of 0.5 roprivacaine. An oblique incision was made over the right inguinal canal and deepened down to the external oblique fascia, which was opened in direction of its fibers. The cremasteric muscle fibers were split longitudinally and a long,indirect sac was dissected away from the cord structures and from the cremaster muscles and from the inguinal floor and dissected into the internal ring. The inguinal floor was also herniated and the posterior wall was opened in the usual fashion useing metzenbaum scissors opening it from the level of the epigastric vessels to the pubis and dissecting the posterior preperitoneal fat away from the posterior surface of the transversalis, which was attenuated . Next,the first layer of the shouldice repair was begun using the lateral transversalis suturing it to the rectus tendon with 0 proline suture and running this toward the internal ring,closing it off and running it back using shelving edge of the inguinal ligament to the internal oblique muscle and tying it at the pubis.A 3rd and 4th row of 2-0 prolene was used to reflect the external oblique overhead inguinal floor. Next,the lateral external oblique was closedwith running 2-0 vicryl suture. Scarpas fascia was closed with running 3-0 vicryl suture. The skin was closed with running subcuticular 4-0 monocryl suture dermabond was applied

Thanks for all the replies. Yes,report says there was a direct and indirect hernia found once I was opened up. Reading the report it doesn’t say any nerves were cut. It does say that the one nerve was injected with a nerve blocker to reduce pain. I also read this is common. The base of the penis and both testicles starting hurting a few days after surgery and still do about the same. Penis turned black and blue which I was told is normal. Color went back to normal after about a week. . It was done in the USA. I’m told that it will get better in time,but I haven’t really seen much improvement in that area yet. Burning pain in my upper thigh running up to area were I was cut was unbearable for about 3 weeks,but that has gotten alot better. Testicles have not.My relative had his done at the same place a few years ago and he didn’t experience any of this. He had one hernia. I know we r all different and I shouldn’t compare his to mine. I have other pain with it in a few spots and a big area of numbness. Still get pain down the inside of the inner thigh and area below incision still seems numb and I have pain sitting down or putting my leg up to far. I’m dealing with those issues,but the testicle pain is much harder to deal with at this point. It makes life very miserable and it makes my whole body feel sick. If u ever been kick down there,then you know what I mean. Some nights I have to take 2 advil just so I can sleep with it. It’s not a real stabing pain. It’s more of a constant medium dull pain. I read that testicles can hurt up to 2 months after this surgery. I’m heading into the 9 week,so I’m right at that 2 month point. So I’m a little concerned.

I’m 8 weeks out and have developed testicle pain after surgery which is making my life very uncomfortable daily. Not a chance of me having sex with this pain as it is now. I’m hoping that time will change this,but I can’t help but be worried.

Hi dave11. I’m 8 weeks out with the shouldice method and still have pain down the inner leg and numbness and pain gets worse when Im talking loud or coughing and walking.Its below incision area down to base of penis over too inner thigh like a triangle area.It has gotten better, but is still there.The worse part now seems to be constant pain in my testicles. It doesn’t go away and is very difficult to fuction with it daily and very hard to sleep. How long was it before you felt normal again and did you have any pain in your testicles.

WOW,thanks for all the replies. I will try to answer as many questions as I can. It was not at that hospital doug.My surgen thinks the nerve is acting up but will get better weekly. At this point he gave me nerve blockers to deal with the pain,but I didn’t like what I read about side affects so I didn’t take them. I’m still on advil. The burning pain was unbelievable for the first 2 weeks,but now it has calmed down alot and is bearable. Now my tescile hurts and from my cut down to the base of my penis and over to my crease and then back over halfway across where my bladder is all numb to the touch. When I touch it and push on it I have no pain,but all numbness,but I feel lots of pain deep inside that area all the time. I know that makes no sensr.I was a big walker before this.I walked 2 hours a day everyday for 25 years. Now I can maybe walk 10 minutes,but pain never let’s up. Sleeping hurts.moving hurts,sitting hurts. Report says second hernia was discovered after I was opened up. It says one direct and one indirect. It also says nerve was injected with stuff to keep the pain down which I looked up and it said it helps with pain after surgery by doing this.Im 3 weeks out this Wed. I know still probably not enough time yet,but sometimes it feels like no improvement. The burning went way down after icing the area 5 times a day everyday But it doesn’t help the whole area numbness and deep pain below it. It feels like there is something in there at times and bump is still there but I’m told that is common too at this point. Sometimes it feels like hernia is back. This pain is much worse then the hernia ever was.

Update. It was a shouldice and a double inguinal hernia on one side. Direct and indirect. Maybe that’s why it’s taking so long to escape this pain. I have lots of faith in the surgeon.Im just looking for some others who may have went through this and how long it took them to recover and anything that helped speed that up.

Hi William. Thanks for replying to me. I’m not sure which one it is. I’m gonna call for copy of report on Monday. Doc just gave me nerve block meds on follow up appointment and said I’m one of the 1% who has pain like this with it after surgery. He says it will keep on getting better weekly,but right now its kinda hard to imagine that. No imaging or suggesting
anything else on his part. Just take pills and wait. I’m very concerned about this even though I know it’s still early post op. -
Hi Thankyou for responding. I had right inguinal hernia no mesh and no nerves cut. I’m told my hole was big..Walking seems to make the pain worse. Before this I would walk 2 hours a day everyday. Now a short distance is hard to do.
