Ejaculation pain after mesh…
I had a left inguinal hernia surgery with mesh 2.5 years ago. After surgery (8 months) I feel pain in my groin and leg (Near groin side) during orgasm. I read several papers. The problem is dysejaculation. Vas deference entrapment in the mesh. Then, the surgeon removed the mesh (1 year after the first surgery). Now I have a big scar tissue inside. My orgasm pain is doubled. Also now I have daily chronic pain in groin. I feel pain during aurosal. The surgeon offer to remove vas deference. But vasectomy may create new problems (Post-vasectomy pain syndrome-Auto immune issues, loss of orgasm sensation, constant testis pain etc). So another option is orchiectomy. But there are cases, feeling the same ejaculation (Dsyejaculation) pain. They received orchiectomy due to testicular cancer.
Below there are some comments from testicular cancer forum. Their pain is the same as dysejaculation pain due to mesh. They don’t have vas deference, testis and spermatic cord. But the same pain. Also, same pain is also seen in some vasectomy cases.
I want to ask, if there is no testicle, no vas deference, and spermatic cord, then why they feel ejaculation pain at the groin and leg?

Ejaculation pain after orchiectomy
- I had a right orchiectomy three years ago. I have numbness, soreness, sharp pains, and burning from ejaculation below the incision towards my right leg. I have had ultrasounds and second opinions only to find nothing. The problem is that yesterday during ejaculation the burning pain doubled me over, it felt like a fire inside, much much worse than usual. I dont know what to do since i dont really feel like giving up sex.
- Wow! Someone else that has the same issue as me! I haven’t been able to find out why this is happening to me as well. I get a severe sharp burning pain that feels like I have taken a knife to the stomach. It feels like it’s on fire and the pain radiates there for quite some time. Doctor thought it was my ilioinguinal nerve but I’m not so sure at this point. I’m still trying to find out what’s wrong.
- I had a radical orchiectomy on the right side. I do sometimes get severe burning pain on the right side in the inguinal region below the scar on ejaculation. Sometimes its severe, sometimes it’s very mild, and most of the time it is nonexistent. I suspect three possibilities in order of likeliness:
1) Neuropathy, damaged nerves and healing nerves can fire unpredictably. A strong stimulus like orgasm might be triggering something that is nerve related only.
2) Vas Deferens spasms during or leading up to ejaculation. The vas deferens is no longer anchored since it was severed during orchiectomy. Perhaps smooth muscle twitches are causing it to move around causing pain.
3) Retrograde ejaculation into the vas deferens may be swelling it or even leaking out into the pelvis if it has recanalized as is common in vasectomies. The swelling from backpressure or leakage may cause pain/burning.- I’m 9 months post testicular cancer. My right testicle was removed during the orchiectomy . Since the surgery, orgasm is the real problem. When I reach orgasm, a warm throbbing rushes over that area (not in a good way!). It hurts and feels like an invisible hand is gripping the area and squeezing hard. I hate it enough that it’s affected my sex life and I hardly even feel like it anymore because I dread the pain. (The pain probably lasts less than 30 seconds, but it knocks the breath out of me.) Has anyone else experienced this?
Ejaculation pain after vasectomy
- I had a closed vasectomy.This past week I also experienced a terrible pain following intercourse which I had never had. It was really bad, with hot flashes, soreness, throbbing, and painful sexual function. When my wife and I played a little too intensely and I had a pain that returned for a month, making arousal and climax painful (as well as movement and daily tasks). I suppose my definition of arousal pain would be pain felt in the genital region when presented with an arousing stimulus and/or during any type of pleasurable sexual contact. As for arousal pain, at its worst, I recall going through the grocery store, seeing someone moderately attractive, and feeling a pain in the epididymis. It was widespread and persistent pain and didn’t take much to trigger it. Orgasms were very painful at that time, with a distinct sharp pain. Your timeframe of discomfort for around 1 hour after is in keeping with what I recall, and I also recall sensitivity in the area that’d last long after that.

Dear Dr. Brown, Dr. Towfigh and other friends.
A urology doctor in my country did a spermatic cord block. After this procedure pubic hair area got numb. During ejaculation i didnt feel any pain.
Dr want to perform spermatic cord denervation. The denervation area is near the testicle (as i see). But my problem is at the deep inguinal ring.
it confused me. dr is very busy and i couldnt ask questions. İs there cases spermatic cord denervation is done at the deep inguinal ring level ???
my pain is in the groin. I received triple neuroctomy and my mesh was removed. Now i have big scar tissue inside. I have ejaculation pain in the groin…… -

Did the skin in the pubic hair area get numb after the nerve block. If it did not, then you did not have a satisfactory nerve block.

Dear DrBrown, Dr. Towfigh and good intentions and other friends.
My pain doctor performed an ilioinguinal nerve block near iliac crest and it didnt helped. After this attempt, he performed another inguinal block and targeted 3 nerves in the inguinal region and also it didnt helped.He concluded that, those 3 nerves are not the source nerves. He didnt injected anything into the spermatic cord. This week i will ask for cord block.
What i want to ask is…… can nerves in the spermatic cord or around vas deference muscles create pain in groin? Are they related?? İ dont have testicular pain. Just pain in groin.
İ feel pain during daily activities, during aurosal and ejaculation. ??
İ am in deep depression…………………….. -

Consider having a spermatic cord block, that might help the pain and avoid more surgery.
The spermatic cord could be adhered to the mesh again.
Skeletonization of the spermatic cord would be the next operation if the spermatic cord block helps.

My first hernia operation………………… December 2016

Steam cell injection in to the groin……..january 2019
Steam cell didnt give relief.My mesh removed completely. İ dont know method of removal. The surgeon performed several mesh removal surgeries. He cut ilioinguinal and genitofemoral nerves ( genital brunch).
İnitial hernia surgery; light mesh was placed. Open and there was no plug. İ dont know direct o r indirect hernia.A pain specialist offer to apply pulsed RF to L1 and L2. İ want to ask, does it give relief…??

Occasionally a spermatic cord block with local anesthetic and steroids can provide some relief. It is a safe procedure and should not make things worse.
Do you still have any mesh?

Can you give details on the type of hernia (direct/indirect), the type of surgery (open or lap), the type of mesh (plug and patch, flat, lightweight, etc.), and the method of removal? Without those details there is not much that anybody can act on. No way to avoid what happened to you.
Nobody can really know/say that all “mesh” is bad or that all “mesh” removal is bad. There are too many combinations of materials and methods, and success stories that show it can work. It’s the details that will allow the bad to be weeded out.
Sorry that you’re having problems, and good luck in finding a solution. Eight months is still a bit early after mesh removal. If things are still changing, it might be best to wait longer. Repeated operations cause more trauma.
