    I was reading a journal article (can’t find it now…) last night in which Shouldice surgeons partly credit their success rate to the extensive dissection done during surgery, which allows them to spot asymptomatic hernias of other types. The question, therefore, is: Could the same be achieved preoperatively with ultrasound imaging?

    It also begs the question of whether some (many?) recurrences are not in fact failures of the repair, but previously undiagnosed hernias of other types becoming symptomatic.

    So far, Dr. Kang is the only surgeon I’ve heard of who does routine imaging for every hernia, even those very obvious on physical examination.
