    Hi, I am a currently very active (XC skiing, mountain biking) 58 year old male who had major abdominal surgery 14 months ago for a unrelated problem. For the last 9 months or so I have been awakened by pain in the penis and left testicle and during stool movement pain can be intense but passes quickly. Surgeon say’s it is Pudendal neuralgia and prescribed therapy which has not helped. Other symptoms are that it can be uncomfortable when in cycling position (aggressive geometry, road racing bike). I am starting to wonder if it is a inguinal hernia and poop moving through triggers pain.

    What was your major abdominal surgery and where were the incisions? That could be a clue as Good Intentions mentioned.

    You can be checked for a hernia relatively easily by exam, but also by ultrasound which may be more definitive if there is some ambiguity. It’s also possible that you may have a hernia but your symptoms are unrelated to it. The groin and pelvis are highly innervated and very complex, there are many different potential causes of pain to rule out.

    As for groin pain and you being a male bicyclist, there could be a relationship there, almost certainly based upon the bike seats compression against very delicate anatomy. Whether you think riding a bike is worth it is entirely up to you, but perhaps try not riding a bike for a few months and seeing if the pain goes away or changes.

    You should describe the major abdominal surgery. There might be clues there. You could also get checked for a hernia.
