    Hi all,

I would to ask experts in here, possibly dr Towfigh on ilioinguinal nerve resection practices and side effects, I was told in a consultation with the surgeon for my inguinal hernia (small hernia, right side) surgery, open repair with mesh, that he may consider to resection of ilioinguinal nerve to avoid future pain or this type of complication.

Is that a normal practice what will be the issues of this resection after the surgery.

    In advance thank you.

Curious as well. I work out quite a bit and decided Lichtenstein would be the best solution for me. I had a positive repair on the opposite side using mesh decade+ prior. Was looking to Dr. Chen in LA. However, transecting nerves gave me pause. I have yet to reach out to his clinic to get additional info on his technique.

Yes. Ilioinguinal Neurectomy performed electively during open inguinal hernia repair is a commonly performed procedure claiming to reduce chronic postoperative pain.

I don’t like it. And I don’t do it. There are risks with neurectomy, including neuroma and chronic pain. Many don’t do the neurectomy correcfly. this practice ignores the many other reasons for chronic pain after open inguinal hernia repair besides ilioinguinal nerve injury.

If you really want to reduce chronic pain, a) make sure a hernia specialist performs your hernia repair and b) consider a preperitoneal (usually laparoscopic) repair.

That’s my thought on the issue.

got it thank you, if I understand correctly you don’t recommend Lichtestein technique which may require neurectomy but the laparoscopic with a mesh that avoids the neurectomy
