Rather long winded story here so bear with me.

male 41 years 185 lbs 6ft 3in skinny muscular pretty healthy, pretty active. Live in Maine USA

My primary care doctor in July of 2016 thought I had a right inguinal hernia said it was very small that he would not refer me to a surgeon as they would not do anything. I was never able to feel it during a self exam and I gave myself lots of them. I had a vasectomy in August 2016 that doctor also felt I did not have a hernia. Had a vasectomy everything went as expected.

Fast Forward December 2016 got hit very hard with a soccer ball in my right groin ( very hard 70+ mph probably ). Didn’t notice any swelling from it although I didn’t look for it. Also got sick about that time frame lots of coughing sneezing. Noticed a squishy area right along my right inguinal canal. No real bump just a doughy area.

Went back to my primary care physician and had another hernia exam he still felt something as well as the doughy area now. I mentioned some groin discomfort at times and he indicated it sounded like post vasectomy pain rather then pain from a hernia.

April 2017 decided to see a surgeon about it to get another opinion. I also was having groin discomfort at times. Based on a physical exam he wasnt sure if I had a hernia although he leaned to me having one. Had a CT scan done. The radiologist report said no hernia detected. The doctor indicated he could see a break in the wall. Doctor and the radiologist talked about it and the story went to I might have hernia. Doctor seemed relieved when I indicated I didn’t want surgery right now.

On my own I decided to have a ultrasound done in August 2017. Surprisingly no doctor would order a ultrasound I had to call like 5 places until one said they would do it without a referral. The ultrasound of me standing bearing down showed what the technician described as fatty tissue or almost like scar tissue. No sign of a hernia though on the ultrasound according to them.

Right now during the day I dont have any pain or discomfort but when I sleep on my right shoulder and tend to wake up every night after about 3 hours with aching in my right groin. Which is quite annoying not being able to sleep soundly.

Some other interesting things I play soccer when Im done playing the squishy spot is gone. Usually it will stay that way for 6-8 hours then be squishy again.

Not sure if I should just wait it out presumably it will get worse if its a hernia. Or go back to the same surgeon or a different one.

I do have concerns over mesh I tend to have skin allergies and hives to everything under the sun. Allergists usually just say that is interesting and it goes away in a couple months on its own.

Have you ever seen someone diagnosed with a hernia only to go under for surgery and find a lipoma instead? Wondering if that’s unusual after a physical exam.

Yes, it’s not uncommon. The fat is plugging the hernia. So,even though a “hole” is not initially observed, once the flap is taken down and fat removed you have a hernia.

Probably a cord lipoma in your inguinal canal.

I can review your CT imaging if you wanted to send it to me. Many surgeons and radiologists don’t appreciate true cord lipomas as being akin to a hernia on CT.

Thanks for the offer. I may take you up on it. Im currently waiting to hear back from bestdoctors as they just got all my information and are going to review it.

Thanks Ill see what I can find in the Boston area which is about 2.5 hours away. Unfortunately theres not much in my immediate area for specialists. If anyone has any recommendations for hernia specialists in the Boston area they would be appreciated.

There is a highly regarded sports hernia surgeon in San Francisco named dr. William brown. You can google him-maybe send your MRI to him for review. He has great bedside manner. Lots of people travel to see him and he is an expert in doing non mesh pure tissue repairs if it turns out you have a hernia.

    Hi, miner

I’m not sure but I think your symptoms are not caused by inguinal hernia or cord lipoma(fatty lump at spermatic cord which resembles hernia), but by the injury to the external aponeurosis of inguinal area (dense and very thin covering which is the continuation of external oblique muscle).

It is called sports hernia and its gross injury is so subtle that it could not be detected and diagnosed by any examination tool such as ultrasonography, CT or even MRI, but only by an experienced surgeon’s hand.

I recommend you to look for sports hernia specialist around.
