    I will try to summarise my situation. I am based in UK and have a bilateral inguinal hernias.
    After a long long research I am still contemplating what should be my next step. It is not an easy task and it is not comparable to taking a haircut 🙂 Like all the patients I want to make a right decision. I am already experiencing other long term and unresolved health issues which may or not contributed to hernia but for sure I don’t need further complications.
    NHS ( national health service) offers only mesh repair which I am definitely not going to accept.
    1st choice: BioHernia which offers non mesh repair in Germany by DR Koch and potentially I could get partial reimbursement by NHS. This is tempting as the procedures are very expensive and despite no one should save on health this isn’t realistic for some of us. I was convinced until I read this forum and especially when coming across comments such as Baris. Therefore are some concerns the way DR Koch actions his decisions and haven’t seen any recent comments about him. On the other hand he has good feedback on Google and Facebook.
    2nd choice: DR Mohan P Desarda website also very convincing but there are high medical costs involved and perhaps exaggerating but I am worried about how good is his performance at the current age.
    3rd option: Dr Kang’s repair. Sounds promising and has sincere responds. Drawback is the medical cost and travel. I can’t stop thinking about the Dr Towfigh’s comparison to Marcy’s technique and not being suitable for adults. Is Kang’s repair any different than Marcy’s ? Any suggestion is welcomed.

    I am personally biased against the Desarda technique. It has not been tested with the rigors of other tissue repairs. There has not been transparency in sharing the data results of Desarda publicly in peer reviewed studies to the extent that there has been for the more traditional repairs, such as Shouldice and Bassini and others. From what I understand, not every surgeon can claim to perform the Desarda technique. Dr Desarda charges to have his hernia technique name marketed by surgeons who provide that technique. I disagree with that approach. I believe that, similar to this forum, efforts to improve patient care should be widely encouraged and free of charge or other constraints to access for the patient.

    Is desarda better repair than Shouldice? According to desarda website it is:)
    But in the opinion of aspecialist like dr Towfigh?

    Wire is pretty benign, sometimes more so than synthetic suture. But it can break.

    I watched some youtube repairs with Shouldice method: there is also going in a lot of suture wire. The body is going to attck this also probably?

    I will try to have an appointment with that doctor and ask if my hernia can be repaired with Shouldice. I doubt it will be possible. Another difference USA/Canada: surgery is done with local sedation, and in Belgium general or spinal.

    I don’t understand the question. He’s on my list as surgeons who can offer Shouldice and other tissue repair options. The statement on his website doesn’t contradict that.

    On the website of his hospital:
    “Although there are different techniques for repairing inguinal hernias without using a prosthesis (also called a “net”), the current guidelines of the European Association for Abdominal Wall Surgery (EHS) prescribe that an inguinal hernia in an adult is best repaired by a prosthesis. abdominal wall prosthesis.”

    Probably no change to have Shouldice there. So why is this doctor on your list.

    You list Muysoms :I search and I find Dr. Filip Muysoms, MD, PhD in Belgium (where I live). Does this doctor could still perform Shouldice?

    Correct. The Shouldice clinic limits its repairs to small to medium sized hernias in normal weight patients, mostly male.

    A tennis ball size is exactly that. If you stand and the hernia is extending well into the scrotum and is larger than a tennis ball size, then tissue repair is not recommended. There just isn’t that much normal tissue to sew together.

    Is there a limit in size of the hernia for tissue repair? Koch: max size of tennisball but how to I have to check the size of my hernia with that description? Maybe the Shouldice clinic gives good results because they only treath small hernia’s.

    I am 12 days post op for a repair by Dr Wiese. He seems to have a very low profile on this forum, and the internet gererally, but my experience has been very positive. He says he does 400 shouldice repairs per annum, and has been doing that for 20 years. He claims to have a a failure rate of less than 1%.

    My recovery has been quick and easy, with little pain. So highly recommended I will do a full write up soon.

    If you want best non-mesh-treatment for your hernia IMHO you should choose Shouldice Clinic in Toronto. Surgeons elsewhere might disagree but a specialized center is always a best option.

    Not that I think it’s funny to spend 4 or 5 days closed there without anything interesting to do. I think smokers can’t even smoke a cigarette. Only some exercising and asking for pain pills.

    But they do the most reliable non-mesh repair, I truly think.

    The fortius clinic would result to me with triple costs of the following surgeons who also operate without mesh in Germany: does anyone have experience for dr Wiese in Kelkeim Frankfurt, besides the already known dr Koch in Cottbus (Leipzig) and dr Lorenz in Berlin? They are equivalent for costs, if they are also for results, it could just be a logistical issue. dr towfight provided a list I could not find
