Jnomesh Thanks for the information. How did the hernia specialist finally concluded that you had a hernia?

The pain today had actually subsided a good amount. I did some stretches last night and have been massaging the area and that seems to be helping a lot. I had been waking up with the pain straight out of bed but this morning I didn’t. But it’s only one day so I don’t know. I have an appointment tomorrow with my primary so hopefully he can give an idea at least on what it could be.

Good intentions I have the operation report. It was 5×5 inches of circular mesh with a coated barrier. And the placement wasn’t TAPP or TEP, it was IPOM so it was placed intraperitoneal up against the peritoneum, if that’s what you mean. So no dissection of the peritoneum was made except for the trocar sites. But yeah I see what you mean. I’ve actually read a few times where that happened and the patient didn’t even know all along and wasn’t even informed. I find it to be shocking to say the least.

Good intentions It was repaired laparoscopically with mesh, the technique was IPOM. Although I don’t believe that the first surgery has to do something with the pain I’m dealing with since the location of the mesh isn’t in contact with anything. But who knows. If it would have something to do with the first surgery, I could only think of the placement of the trocar site maybe? One trocar was placed in my lower left abdomen. But I would assume that if it was because of that then I would have felt the pain right after surgery. But again, what do I know.

Good intentions Thank you for the reply. I’m really hoping it isn’t a hernia. I just had an umbilical repaired a couple of months back that I’m having minor problems with.

I have an appointment with my primary on Tuesday so hopefully he can give me an idea of what it could be. Usually during physical activity it doesn’t bother me but it doesn’t bother me either the next day after it. It kind of just lingers everyday. And I am in Texas.

Here’s a photo of where the pain is. The circled area is where most of it is located and it’s also the area of that “cord structure” that I mentioned.

drtowfigh reason I ask because of adhesions. I know adhesions will always form at the site of surgery and they are nearly preventable, but since the damage of the peritoneum when removing ipom mesh is the same as an incision, do the adhesions usually form much more dense and severe since it’s a big area of peritoneum being damaged (because of the mesh) rather than a line (incision)?

drtowfigh in your experience or any that you know of, have you seen any obstructions with the bowel or intestinal problems AFTER removing ipom mesh since it damages the peritoneum? my mesh is pretty large

drtowfigh is there usually scar tissue left over the defect after ipom mesh is removed?

and when ipom mesh is removed, since the ingrowth is the peritoneum, is the damaging of the peritoneum that is done when removing the mesh, is it the same as an actual incision to the peritoneum (trocar site or open incision)?

Good intentions Thank you, I sent you a message regarding a few questions since I know you went through removal as

idoncov Nope, no recurrence. since it was placed ipom i’m thinking that some scar tissue will still be present? If it does return then I’ll for sure repair it with tissue repair. The defect was not closed tho prior to mesh placement so I don’t know if that will impact wether it comes back or not majorly…

drtowfigh and when you say the only way to place mesh away from intestines is retro muscular after TAPP ventral removal, is that done by conversation to open surgery after removal or is it done laparoscopically as well?

drtowfigh glad to hear that… do you have experience when it comes to positioning the mesh retro muscularly after an ipom mesh removal?

and are most ipom mesh repairs successful from what you’ve seen??

drtowfigh size of the hole was a little under two cm. not sure on mesh I just know it was circular and about 5×5. as far factors I am 5’9 and have a bmi of 19-20. only physical activity I do is running and yoga. don’t smoke either.

Good intentions or really any tissue repair would be fine as long as it’s done by someone who has done a lot and has good experience. i’m thin and had a small hernia so that’s why I think sutures would have been fine. problems are lack of healing it seems like. its been three months since I had the surgery and the site is still really swollen and super firm it’s almost like my umbilical area doesn’t even move freely anymore as far as the insides. also experiencing significant fatigue and the mesh is making me psychologically distressed knowing it’s in there… not saying removal is needed right now but I guess just in case. if I could I would have it removed and have a tissue repair instead

DrBrown Thank you for the reply and information Dr. Brown.

I also should have added that IF removal is done, I would like a pure tissue repair, but a good tissue repair. But since the mesh was implanted laparoscopically I know that it will be difficult to remove the mesh using open surgery method and possibly making the defect even larger. I had a small defect by the way. Is there any possible way I can get the mesh removed laparoscopically or robotically assisted so the defect won’t get larger, then followed by a repair such a Shouldice repair or Desarda repair? The question is for anyone to answer

Forgot to add, along with doing a pure tissue repair.
