Removing the mesh is a big decision.
Work with your surgeon to have everything check before you consider removal.
You reported that you had many of this problems before surgery. So removal of the mesh may not help.

Your best chance to find a local surgeon who can do a non mesh repair is to consult with a surgeon who is at least 60 years old. He/she would have been trained before mesh was widely available.
The weather is pretty nice in California if you wish to make the trip.

A Shouldice is used for inguinal hernia repairs.
A simple suture closure is all that is needed for an UH.

I have not had much success with Lyrica.
A nerve block with steroids will have a much better chance to help you.
Nerve pain is slow to resolve. 6 months is a more likely duration.

Ask for a standing ultrasound with valsalva. That is the best test to evaluate for a femoral or an inguinal hernia.

I have not had very good results with Lyrica.
Nerve blocks with local anesthetic and a steroid can be very beneficial and provide longterm relief.
There is a good chance that the pain will resolve with time, but it will take longer than 4 weeks.
4 to 6 months is a better estimate.

Dear Luke.
Most likely the mesh is wrapped too tight around the spermatic cord, or the cord has become scarred to the mesh.
A Spermatic cord block may provide some relief.
Sometimes injecting the mesh with steroids will provide relief.
But there is a high chance that you will want the mesh removed.

Since your changes started after the operation, it is probably related to the operation.
Since the changes are not causing your much trouble, I would not advise you to have anything done at this time.
Wait until you have healed from the mesh removal operation.

Dear Double.
Ask your local surgeon to inject the mesh with a local anesthetic. If you feel better for a few hours then that would be strong evidence that the mesh is the source of your pain. Then you can decide whether or not you want the mesh removed.

Everyone heals at different rates.
As long as you feel like you are making progress, then be patient.
Tissue healing is usually complete at three to six months.
Work with your surgeon and your physical therapist.

Dear Andrew.
From your description, you allodynia related to an injury to the ilioinguinal nerve.
A nerve block can be very beneficial.
Neurontin can occasionally help.
Capsaicin cream can occasionally help.
Nerve ablation can occasionally help.
There is a chance that this will improve with time.
You may need a neurectomy of the ilioinguinal and the iliohypogastric nerve.

I think that the recurrence rate is more dependent on the surgeon than on the suture.

I remove mesh through an open incision, but the robotic is also an excellent choice.
After the surgery, you should be able to return to work in two weeks.
There is a good chance that Dr Belyanksy will not need to add mesh.

Slowly increase your activity.
You surgeon can inject the mesh with steroids, that will often soften the mesh and decrease your pain.
The pain should also slowly improve with time.

Silk is a natural fiber. There is minimal foriegn body response by the body.

The major purpose of sutures is to hold the tissues together until the body heals the tissues together.
The final strength is dependent more on the body’s healing than the sutures.
At three weeks a wound has about 70% of the final strength.
At six weeks a wound has about 90% of the final strength.
It is not until about 12 weeks before there is 100% strength.
Thus the ideal suture is one that keeps its strength for at least three months.
I use silk sutures because it maintains excellent strength for six months and then slowly dissolves.
Silk also allows the tension to be set at just the right amount.
