    That was a pretty informative session with Dr Vahedifar. We discussed mostly how to manage nerve pain, including discussions about nerve blocks, ablation options, and spinal stimulants. I shared data from our most recent research on outcomes after neurectomy—-hopefully soon to be published. We also discussed the use of PRP and athletic pubalgia.

    I believe the literature shows the recommendation to be u see 25 CT scans before there is a risk of radiation-induced malignancy.

    I am personally biased against the Desarda technique. It has not been tested with the rigors of other tissue repairs. There has not been transparency in sharing the data results of Desarda publicly in peer reviewed studies to the extent that there has been for the more traditional repairs, such as Shouldice and Bassini and others. From what I understand, not every surgeon can claim to perform the Desarda technique. Dr Desarda charges to have his hernia technique name marketed by surgeons who provide that technique. I disagree with that approach. I believe that, similar to this forum, efforts to improve patient care should be widely encouraged and free of charge or other constraints to access for the patient.

    Desarda is a poorly studied technique. The results are highly variable from surgeon to surgeon and not reproducible.

    I personally am biased against the repair. There are much older and better studied repair options for inguinal hernias, such as the Shouldice, Bassini, McVay, Marcy, etc. Surgeons who were performing a surgery similar to the Desarda back in the 1950s abandoned the technique because of the high failure rate.

    This was such a great hour! We discussed anything and everything you need to know to have a safe and low risk anesthesia for your hernia surgery. These included topics of general anesthesia, IV sedation, regional anesthesia pros and cons. We also discussed what medications to take and which medications and supplements to prevent before anesthesia to get the best outcomes.

    Wire is pretty benign, sometimes more so than synthetic suture. But it can break.

    This is true. Some times genital branch neurectomy can cause hanging of the testicle. It’s not 100%. It may be closer to 1-2%, as the original Shouldice technique and others routinely cut that nerve.

    The treatment of the hanging testicle, however, is not orchiectomy. It’s a cremasteric pull-up procedure, where the muscle is pulled up and sutured to the pubic tubercle. I would question any plan for orchiectomy.
