    All hernias are at or above the groin crease. The pain may radiate down the thigh but there should be no bulging or hernia at the thigh.

    Every hernia repair has an average recurrence rate associated with it. Usually recurrence is related to the type of repair and mostly patient factors, such as cough, straining, nicotine use, obesity, wound healing issues.

    See pinned post re AHSQC. It’s the only effort in US to get longterm mesh specific and repair specific data. We need more surgeons to participate. Plus we need patients to log in their longterm data.
    danish and Swedish and German registries are pretty robust for longterm data and they publish their results.

    See the pinned post on the AHSQC. I highly encourage everyone to see doctors who are members and participate in this and also to respond to the longterm questionnaires for this.

    Some doctors have their own system for followup. But it’s usually a problem.

    Simple allergies likely don’t make you at higher risk for mesh reaction or mesh allergy. Urticaria May place you at higher risk for mesh reaction or mesh allergy.

    in situations where I feel the mesh may spark an autoimmune or auto inflammatory syndrome or allergic reaction, though rare, then I use hybrid mesh that is much less inflammatory

    Mesh recalls to date, with two exceptions, have been for problems of packaging and other things unrelated to safety of the mesh.

    for the Composix mesh, it was recalled because of risk of breakage and intestinal injury. No patient was required to have the mesh prophylactically removed. For the Physiomesh, there was a higher recurrence rate when used as a bridging method and so it was removed from the market. Again, no mesh required prophylactic removal.

    Laparoscopy repair is the best option for most athletes.

    my reference is to pure biologic mesh. I am a fan, however, of hybrid mesh, such as Tela Bio’s Ovitex. It has 4% synthetic component which seems to be enough to prevent a hernia recurrence but not enough to instigate an inflammatory or autoimmune response.

    For the most part, with some technical details being left out, direct and indirect hernias are treated similarly, with mesh and without mesh.
    laparoscopic suturing is not performed as the angle and technical expertise required is complex.
    robotically, I do perform suture only hernia repairs
