Couple of comments:
– Dr Victor Radu is the most talented surgeon I know in Romania. Not sure who your surgeon was.
– Lateral hernias are a different situation than midline hernias. The muscles are weaker. Hernia repair outcomes are worse. I’m curious how your repair was secured. Eg, we have moved away from pinning sutures into the bone. A TAR was likely performed because the defect was too wide to close without a TAR. Do you know how wide the defect was?
– Lateral/flank repairs will feel tighter than the opposite side. Fortunately you had lightweight mesh and it was placed retromuscular. That’s the best option.I’d review the operative report to see the technical issue that may be contributing to pain. Perhaps some sutures can be removed. Mesh removal will be difficult and likely result in worse outcomes. Physical therapy and even neurotoxin injections may help.
The surgeons mentioned focus best on groin and I doubt they would be able to help you.

This is often the cause of the bulging seen with hernias. The fat comes from the insides (retroperitoneally) and is really the herniation of preperitoneal fat. It is rarely just a lipoma unattached to anything, like one sees on an arm, leg, or back. The name is really a misnomer. In rare occasions the lipoma is a real lipoma or liposarcoma (malignant tumor) and not a hernia.

But that history is important to share with your surgeon, as there are risks, especially with laparoscopic surgery, in the face of prior infection and scarring from diverticulitis.

It is a skiing theme and very family friendly, which is nice to see for a surgical meeting. The scientific sessions therefore start at 6am and the room is packed. This is because the topics are the most impactful and insightful of any meeting.
Dr. Novitsky has been running this since before he started his new position in Columbia Hospital.
Dr. Felix, as you may know, was trained during the tissue-based repair. So he is an excellent surgeon to discuss this. Even more relevantly, his professor was Dr. Nyhus, who of course invented the Nyhus tissue based repair. This was the open precursor to the laparoscopic TEP inguinal hernia repair. Dr. Felix is a master innovator in all things laparoscopic, not just inguinal hernia. And we surgeons are not single hit ponies. We are able to invent one operation type but still perform others.

@dm249, there are multiple reasons for this.
When the testicle was moving up, this was the first sign that your inguinal hernia repair had recurred. Once it was repaired, then the question is:
– is the testicle retracting because it’s used to being higher in the canal, and the spermatic cord is foreshortened, so now that there is a mesh holding part of the spermatic cord in place, the testicle doesn’t reach lower? There is a treatment for this. It can include dissecting the mesh off the spermatic cord or performing an orchiopexy. -
