Your surgeon should help figure out if your symptoms are due to recurrence of the hernia or from the mesh or both.
Removal of mesh that was placed laparoscopically is no small feat.
In Germany there are excellent surgeons for hernia related pain. You are right that the most famous ones do only open procedures. But they can help you figure out why you have the pain. If they feel that the mesh is an issue, then they should be able to help in that realm too.Some names to consider:

I cannot say definitively, but perineal pain, which is what you seem to be describing, is usually not a result of inguinal hernias.
Sometimes, obturator hernias can cause this symptoms. Sometimes, pudendal neuralgia can cause it. The pudendal nerve may be entrapped by scar tissue, if you had surgery there, it could be directly injured, such as with pressure from a bicycle seat, or may be entrapped by spasms of pelvic floor muscles, hamstring, etc.
So, perhaps you have hamstring or hip injury or strain that is causing secondary pain.

Both are excellent surgeons for mesh removal for the abdominal wall, if necessary. Dr. Belyansky is also highly skilled and experienced in mesh removal for the groin region.
I am unclear as to exactly what type of hernia you had repaired and where the mesh was placed for your abdominal wall reconstruction. That said, both Drs. Bittner and Belyansky would be skilled to handle the issue and I would highly recommend them and trust their recommendation.

Drs Bittner and Belyansky are also super gifted and talented surgeons for hernias. I was referring to inguinal hernias, not abdominal wall, and would add Dr Belyansky to the list for inguinals.

That’s a question that needs to be answered based on each patient’s specific needs and risk factors. Rely on your surgeon to make the right recommendations for you.

Of course everyone is different.
The vast majority of patients have resolution of pain during the recovery period. That is typically within the first 6 weeks and for sure by 3 months. We usually get concerned with pain lingering after 3 months, which can happen in a fraction of patients. -
Have had pain for about 3 months
Most patients are symmetric. And we sometimes see new symptoms on the opposite side when one side is repaired. The pain from the one side is no longer masking the lesser pain on the other side.

MRI may show residual mesh. Though surgeons estimate the amount of mesh they remove, it is by no means an exact estimate, so it can still be the mesh, as Dr Ramaswamy suggested.

Hernia mesh and low testosterone
Mesh or hernia repair does not affect testosterone level. Sex drive may be related to the level of testosterone.
Other symptoms need to be carefully evaluated to determine true cause and tailored approach to addressing it. One size does not fit all.

Yes, but with much higher rate of postoperative pain, longer recovery, higher risks for chronic pain and also higher recurrence rate. It is far from the gold standard.

No referral. Just call to make an appointment. The doctor’s office should take care of the rest or may seek a referral from your medical doctor. It depends on your insurance and the doctor.
I recommend Ramshaw, Chen, or myself as we have the largest experience in the nation if not the world.
Depending on the situation, I work with a urologist and I prefer to perform mesh removals with robotic assistance as my research study has shown that my results in terms of cure is best using that technique.
There is a hope for cure.

Wow.
Ok, lots to discuss here.
First comment: you are at very very low risk of losing your testicle and virtually no risk of losing your leg if mesh removal is performed, especially by a skilled laparoscopic or robotic surgeon who has experience.
It’s a very complex algorithm to follow to determine exactly why you have pain. My hunch is you may have an obstruction of your spermatic cord by the mesh placement. I’ve seen it before. Mesh removal is a cure.
Other things to rule out include hydrocele, varicocele.
This does not on its own sound like a nerve issue.
You really need to see a specialist for this problem otherwise you are at risk of delay in diagnosis, care, and significant decrease in your quality of life.
What is your situation in terms of which civilian doctors you can seek attention from? I know of great hernia surgeons at the VA but they don’t have experience in revisional or complex operations of this level.

The side bruise looks like site of an injection. Or perhaps bumped somewhere. Doubt it’s related to the operation.
