As I’m reading your story I recall how many hundreds of times I’ve heard the same story, and a hernia was the cause of the chronic pelvic pain. In my patients, they never really had the diagnoses they were labeled with, such as IC and IBS. It just led them astray and delayed their diagnosis and therefore treatment. Many of my patients had hysterectomies unnecessarily. I even had a 20-year old female last week who was told she needs a hysterectomy for her chronic pelvic pain and endometriosis. I fixed her hernia. She’s pain free after 8 years.
Hernias can be painful. Size is not a determinant of the pain. If anything, smaller hernias are more likely to hurt than larger hernias.
Femoral hernias should be repaired regardless of symptoms. Laparoscopic repair with mesh is the gold standard.
I know your NY hernia and sports hernia specialists. They are excellent. Stick with them. Your investment will be well worth it.

Having multiple hernias can be difficult. It’s also so important that you correct all risk factors for hernia recurrence prior to undergoing another operation.
Yes, weight loss is critical, as it can contribute to hernia recurrence.
Correct any constipation. So important. Treat it like you would any other disease. This means you may have to take a medication daily.
Exercise to strengthen the abdominal core muscles will help you.
I agree with you: it’s so ridiculous that insurance companies only approve a same day surgery for an incisional hernia repair. Almost all of them will require a hospital stay. That is a big and complex operation. This is just the insurance company’s way of having the opportunity to deny any hospital stay. Fortunately, they usually don’t. So don’t worry. As long as your surgeon documents why you’re in the hospital that should be ok

Bowel issues with Hernia pain
Do NOT push or strain. That is the worst for hernias. It will increase abdominal pressure and cause hernias to recur.
That said, if you are missing Core abdominal function, it is possible that bowel function may be difficult, because you cannot make a concerted effort to push stool down into the pelvis and evacuate and you are completely dependent on colonic motility alone.
A strict bowel regimen is imperative so that stool is soft and not hard. Can include mineral oil, magnesium mallate, Miralax, milk of magnesia, prunes, water.

Membership in AHS does not imply specialty in hernia. At the least, there is an interest in hernias. The reverse may not be true. I would question a hernia specialist who has not taken the initiative to be an active member of the AHS and also participate in the AHSQC.One clarification about residents and their role in the surgeon’s experience. Residents do not perform surgery on their own. They are under the direct supervision of an Attending surgeon. Having a resident participate in an operation does not in any way reduce the amount of skill or experience of the surgeon. Quite the opposite.
The surgeon must be present and typically scrubbed in for the important part of the operation. Allowing residents to participate in one’s operation takes a lot of skill on the part surgeon.

I disagree with the statement that it is unsafe to remove mesh. It for sure has its complications. Thus, best outcomes are with those surgeons who perform this procedure on a regular basis.

Yes, it is possible that an inguinal hernia is missed on laparoscopy alone. That said, the chance is lower in men, as women tend to have occult hernias much more than women. Also, I am. It sure what your specific situation may be

Have had pain for about 3 months
You don’t have to have a bulge to have a hernia. In women, occult or hidden hernias are more likely. And they hurt.
CT scan is often incorrectly negative for hernias. Dynamic ultrasound or MRI pelvis is a better imaging modality to help diagnose this. -
