    Certainly having multiple different hernias in different places may suggest a genetic disposition and in rare cases a connective tissue disorder.

    It seems however hat your hernias are all related to your groin and/or c-section.

    Pain from hernia repair may be due to recurrence of due to the repair itself. It may be a nerve impingement, too tight of a repair, tearing of the repair, and so on. It all has to do with where the pain is and what activities make it worse.

    CT scan is best for abdominal wall imaging, but not as good for the pelvis and groin area.

    Contrast study with oral contrast helps show Bowel vs muscle/mesh. The IV contrast helps show inflammation, infection. In some cases it may help show mesh vs muscle. It is not absolutely necessary.

    I presented my data at a recent surgical society conference. Over 80% of patients had cure of their pre operative symptoms without need for further procedures.

    We surgeons are also frustrated by inadequate or incorrect documentation. You are not alone.
    In important situations, we can call the surgeon to clarify the records. In the best case scenario the medical record may also be corrected. It’s rare.

    The Kugel repair has been abandoned by many for different reasons:
    – if the space is not adequately dissected, the mesh may fold or crumble and cause meshoma with mesh-related chronic pain. Then it’s difficult to undo.
    – if the dissection of the hernia is not wide enough, hernia recurrence is higher
    – the mesh product that gave the most success is considered heavyweight and the ring is also considered too stiff–even the newer generation product.
    – repair are not promoting the Mesh used for Kugel repair.

    – risks and benefits of anesthesia options are variable for each patient. Usually risks are related to increasing age.

    – the Kugel technique is a very good technique. It’s fallen out of favor as less surgeons are performing it. I use it for only certain situations. It’s good to have in one’s armamentarium.

    – yes. The round ligament is routinely cut during many Inguinal hernia operations. No problems.

    – short answer is no, laparoscopic non-mesh repair is not possible. I do offer a robotic laparoscopic non-mesh repair

    It’s possible that the underlying pathology that led you to need a rectopexy also caused a hernia, that is, straining to have a Bowel movement. In other words, it was t from the MRI or proctogram.

    Yes, hernia can be missed on ultrasound. Should be there on your small bowel MRI, though. And yes, your symptoms can be due to inguinal hernia.

    All the symptoms you describe may be coming from your umbilical hernia. That said, the main purpose of hernia repair is to improve your quality of life. Size is almost never a primary determinant of repair. If the symptoms are bothering you, then repair can help.

    No perfect answer to this question
    For most, mesh repair is best
    For some, nonmesh repair is best
    Not all mesh repairs are the same. The risk profiles for pain are technique dependent, mesh-dependent, and patient-dependent.
    At this time we don’t know what is the perfect repair for each individual. We can Naif surmise based on population data. Dr Ramshaw is leading the cause to perfect this tailoring.
    And yes, there is significant risk for non-mesh repair as well. It just wasn’t reported in the past. And is more obvious in the modern data.

    Make sure the laparoscopic exploration includes takedown of the peritoneum if normal laparoscopy does not show an obvious hernia.
