    Epidural can help if nerve blocks didn’t and it’s a nerve issue.

    Otherwise, it’s not a nerve issue.

    Ultrasound being negative, especially if performed without any maneuvers, has a very high false negative rate. Repeat ultrasound with valsalva or CT with valsalva or MRI pelvis should help.

    Neither. Both are equally challenging.
    Depends on how the mesh was implanted. In my practice I remove the mesh based on how it was placed, i.e., lap vs open.

    Patients should advocate for their care. In fact, insurers are more likely to listen to patients than doctors. Yes, it’s true!

    So make a big stink about your situation. Have your doctor do a primary and secondary peer review. Write letters to your congressman, the Insurance Commissioner for your state, and whichever congressman sits on that committee. Write public reviews. Better business bureau. Yelp. Whatever it takes.

    It is horrible that pain control is considered not indicated. It’s just not true.

    I respectfully disagree with the 6-weeks. There is no evidence to support inactivity, including sex, until 6 weeks postop for inguinal hernia repairs. Especially primary non recurrent ones and laparoscopic.

    I don’t restrict my patients on any activity. This is also supported practice by the European Hernia Society consensus statement.

    Pain with ejaculation and orgasm may be related to mesh:vas deferens interface. It should resolve with time. If it doesn’t, see a specialist. Rarely, the mesh may need to be lifts off the spermatic cord.

    Otherwise there is no correlation.

    Desarda technique has been used by Dr. Bob Burns in Newport Beach. I am not sure if he is still practicing.

    The tissue repair is performed by many of us in L.A. area: Myself, Dr. David Chen, Dr. Edward Phillips.

    I also agree that Dr. Johnson in Palm Springs is a good option.

    Agreed.

    If the hernia has a (large) scrotal component/extension to it, it is good practice to leave the deeper extension of the sac alone. Otherwise, the operation becomes more likely to bleed, cause swelling, and even a hematoma.

    Black and blue discoloration occurs at about 3-5 days after groin surgery and lasts usually about 1 week and then resolves.

    This is basically blood from the operation. The more dissection, bleeding, scar, etc., from the operation, then the more likely to have the discoloration. Usually, larger hernias are more likely to have postoperative discoloration. The color is because the blood at the level of the hernia repair/dissection then travels through the tissues as it is pulling from gravity and ends up wherever gravity takes it. It does not imply anything more than it was a difficult/complicated/large/bloody/scarred area.

    There are a handful of surgeons who perform non mesh hernia repairs. There are also a couple who perform the Desarda technique.

    I’m not sure if your husband is a candidate for tissue non-mesh repair. That would’ve a discussion to have with your surgeon.

    What insurance plan does he have and where in California do you live?

    Hernia is big but not that bad.
    Congrats On the weight loss. Every pound helps.
    The rectus muscles are retracted. In my practice I would inject Botox to lengthen those prior to surgery.

    Very important questions.

    – read through HerniaTalk to learn about risk factors for recurrence. That has a long answer. Typically it’s constipation, chronic cough, obesity. Your surgeon can tailor this answer based on your needs and situation.

    – perhaps. There are conflicting studies that show reduction or no effect on sperm count after mesh hernia repair. The most recent studies say no difference.

    – no restrictions

    – risks of open surgery are long as with any surgery. Read or search through HerniaTalk or visit your local surgeon for answers.

    Sorry to hear about your story.
    Yes, imaging is very helpful. MRI is my imaging of choice as it often shows the mesh. It will also assess its placement, it’s relationship with respect for the spermatic cord and other structures, and would show a hernia recurrence if there is one.
