    Thanks to Bill Brown, M.D. for the clarification. At the risk of being repetitious, I cannot emphasize enough the many research articles dealing with cognitive impairment following general anesthesia. This is enough to avoid Laparoscopic methods. This is especially the case for patients already at risk for cognitive decline. Neurologists will tell you that G.A. is NOT good for the brain. In my opinion, if a surgeon is afraid to use local when the hernia is non-complicated, you should avoid that surgeon as he probably needs to take some classes in anesthesia. This assumes the patient is not overly anxious and might freak out during surgery. I had one surgeon agree to do with with G.A. backup. Once your brain goes the hernia becomes the least of your problems.

    If that was the case, how do you explain the original Desarda video in which he asks the elderly patient under local to cough to test the sturdiness of his repair? Does not sound to me there is an issue with abdominal movement.

    I will also add there is a lot of research showing subtle brain damage from general anesthesia that is usually manifested initally as memory problems. That is a very good reason to go local.

    Not sure about google,I would have to research that. Was not singling out Indiana Hernia Center except that his reviews are uniformly very high with very little negative which,in itself, makes it suspect. Another is the surgeon in Sarasota Florida, no negative reviews.
    I think all it really takes to get rid of a negative review is his attorney threatening action and the site will fold. That plus the fact that many of the reviews sound so phoney (‘oh the best surgeon in the world’ etc.) I did find one review of the Indiana Center which accused the surgeon there of sexually inappropriate behavior, but really, the net is so hacked and some people abuse their access who knows if it has any validity? I think he is a good surgeon as far as surgeons go. My biggest gripe of surgeons in general is that they refuse mostly to take responsibility for bad outcomes.

    good intentions-interesting reply I had not thought of. Frankly I think they just are not comfortable with the patient being awake-they are not in complete control.

    Honestly I cannot think of ANY positives for general anesthesia for open uncomplicated repair. What positives did he tell you for that?

    The website this podcast is supposedly hosting does not work properly. Had a hell of a time getting the short clip to download or run.
    That being said, not sure Nader knows what he is talking about. The research does not show excessive problems with mesh but there sure are a lot of patients complaining post op. Also Nader was a patient at Shouldice so he has a conflict of interest.

    Can anyone else confirm this? The two surgeons I had phone consults with did not want to do it. One said he would do it IF they had backup general waiting.

    Did you go back to the surgeon and what did he say? As if it matters they hardly ever take responsibilities for bad outcomes.

    Why did you have your surgery done in the Philippines? Not noted for it’s good medical. What hospital did you use, some are better than others, but none except at St Lukes possibly come up to U.S. standards. How can you expect someone to diagnose you over the net who has never seen you? Hernia surgery is not simple like many people think. Lots of mistakes and complications reported.

    Much thanks to Bill Brown, M.D. for the comprehensive look at the various techniques. When I sold to orthopedic surgeons I learned that the skill of the surgeon is the most important factor in determining a good outcome. Can you kindly advise where you are located, any contact info and if you are taking new patients? Thanks.
